Our services support the transition from the facility to the planned recovery setting by reinforcing existing discharge plans and providing recovery-supportive coordination.

Three simple steps....

Request Your Date
A patient, family member, or authorized representative submits the scheduled procedure date and basic contact and service details to request availability. A date request is not yet a confirmed booking.

Complete Your Booking
If the requested date is available, PrimeVera Recovery sends an email with a secure intake form and payment link. The completed intake form and a 50% deposit are required to complete the booking. Once the intake has been reviewed, the request accepted, and the deposit received, PrimeVera sends written booking confirmation.

Premier Recovery
On the scheduled visit day, services begin only after the patient has been formally discharged from a licensed healthcare facility and written discharge documentation is available. Services are then delivered in the planned home, apartment, or hotel recovery setting within defined scope and service parameters and coordinated with existing discharge instructions.
Before Services Begin
PrimeVera Recovery requires a formal discharge from licensed healthcare facility and access to the patient written discharge documentation and instructions. Services do not begin when a patient left or signed our against medical advice.
Date availability alone is not a confirmed booking. Primevera sends written confirmation after the intake form has been answered.
PrimeVera Recovery does not provide medical diagnosis, treatment, or emergency services. Medical questions should be directed to a licensed healthcare provider. In an emergency, call 911.
Formal discharge is required before service begins.
PrimeVera Recovery does not initiate service when a patient has not been formally discharged or has left or signed out against medical advice. Required discharge documentation and instructions must be available before services begin.
PrimeVera Recovery serves as a post-discharge recovery support resource for healthcare professionals and care teams. Services operate after formal discharge, align with existing care plans and written discharge instructions, and maintain clear professional boundaries.
